This Extension of Complaint Form ("Agreement") is made and entered into as of the ___ day of _______. by and between:

WHEREAS, the Client has submitted a complaint to Sparkly Housekeepers and is requesting an extension of time to further address the complaint;

 WHEREAS, the Client acknowledges that the request for an extension is due to being out of town for the reason of: NOW, 

THEREFORE, in consideration of the mutual covenants and agreements contained herein, the parties agree as follows: Extension of Time the Client is hereby granted an extension of ___ days from the original deadline to address the complaint. 

The new deadline for submitting any further documentation or response related to the complaint shall be ___ day of _______.

 Notification Requirement 

If the Client requires additional time beyond the extension granted herein, the Client must contact Sparkly Housekeepers via email at [insert email address] no less 1 than seventy-two (72) hours prior to the new deadline to request additional time.

 Failure to comply with this notification requirement may result in denial of any further extension requests.

 Acceptance or Denial of Request 

Sparkly Housekeepers reserves the right to accept or deny any extension request made by the Client. The decision shall be communicated to the Client via email.

 Accepted Request: 

If the request is accepted, the Client will be notified of the new deadline for the complaint submission.

 Denied Request: 

If the request is denied, the Client will receive a written notification detailing the reason for denial. Reason for Denial (if applicable): Finality of Extensions for complaints shall end on the ___ day of _______, and no further extensions will be granted after this date.

 Governing Law 

This Agreement shall be governed by and construed in accordance with the laws of the State of Wisconsin.

 Acknowledgment

 By signing below, the Client acknowledges that they have read and understood this Agreement and agree to its terms.
